Field Case Manager - Family Care Partnership (FCP)

Humana/iCare in Madison, WI seeks a Case Manager to provide care coordination for adults with disabilities and frail elderly under the FCP program. You will perform assessments, plan care, coordinate services, and monitor outcomes with an interdisciplinary team. The role requires travel within Dane County and surrounding areas up to 75%.
