Responsible for assessing, observing, evaluating, and documenting patient interventions; providing teaching, counseling, and support; and leading a team to ensure optimal patient care. Must be familiar with community health resources, social, and welfare services. Duties and Responsibilities Takes patient history, vitals, and symptom documentation; administers screening tests. Demonstrates leadership and supervisory ability; works harmoniously with professional and non‑professional personnel. Administers patient immunizations per protocols and provider instructions. Administers medications and treatments; ensures functionality of equipment, maintains inventory, and orders supplies within budget. Performs lab duties: quality checks, specimen processing, documentation, cleaning, and disinfection; submits monthly logs. Ensures clinical competency of supervised staff; provides individual or group health education and distributes materials as needed. Reviews chart documentation for completeness; creates schedules; contributes to performance evaluations. Travels to other Centro Del Barrio sites as required. Instructs staff on infection control, nursing policies, hazardous materials, and safety procedures. Delegates and assigns duties within scope of practice and company policies. Works with patients aged 0 and older. Participates in the agency’s quality improvement program. Performs other duties as required.
