MDS Coordinator

Summary Under the direction of the DON, the MDS/RAC is responsible for conducting and coordinating the completion of the resident comprehensive assessment (RAC) which includes the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and the RAP summary sheet. The coordination of periodical comprehensive, interdisciplinary assessments upon admissions, annually and whenever there is a significant change of condition. Location: Sunny View Retirement Community Hourly Pay Range: $55.00 - $65.00 Work Schedule: Monday–Friday Detailed Responsibilities Provides quality customer service to residents, families, co-workers, and vendors to ensure satisfaction. Ensures interdisciplinary, comprehensive assessments and care plans are completed within the time frames established by OBRA regulation. Schedules interdisciplinary conferences for review and updating of resident assessment and care plan. Monitors plans of care to ensure resident assessments are used in the development of an interdisciplinary plan reflective of the resident’s current physical, mental and psychosocial needs. Provides educational training and clinical supervision in resident assessment for appropriate health care workers. Assists Resident Assessment Coordinators with computerized medical data and transmission. Ensures documentation relative to resident assessment process (RAI) is maintained according to state and federal regulations and company policy. Communicates problem identification promptly to the DON and Executive Director. Conducts and participates in PDPM meetings. Reports data collection, trends, QSAP reports and attends QA meetings. Performs all duties safely and efficiently, wearing required safety equipment and reporting hazards or accidents to supervisor. Attends all mandatory in‑service meetings and complies with department and facilities policies. Ensures safe work habits to prevent injuries to employees, residents, visitors and other staff. Essential Competencies & Skills Good communication and customer service skills; ability to work well with others and take direction. Ability to work under pressure to ensure timely review, correction and reporting of payroll data. Assists accounts payable and accounts receivable with data entry, bank deposits, etc. Essential Credentials Registered Nurse – California license.
