Referral Coordinator

We are seeking a Referral Coordinator to support a senior patient population by coordinating referrals and helping members access the specialty care they need. This role is heavily focused on scheduling appointments, patient outreach, communicating with specialist offices, and ensuring referrals are completed and properly documented. The ideal candidate is organized, detail-oriented, comfortable spending much of the day on the phone, and able to manage multiple referrals and patient needs at once. Responsibilities Coordinate referrals from primary care providers to specialists and external healthcare providers Schedule specialist appointments for patients, including appointments with cardiology, oncology, and other specialty services Conduct phone outreach to patients regarding upcoming appointments and overdue preventive screenings Follow up with patients who have missed or need to reschedule appointments Contact outside provider offices to coordinate referrals, appointments, records, and follow-up documentation Serve as a point of contact for patients and family members with questions regarding referrals and upcoming appointments Communicate with patients by phone to provide appointment information and assist with navigating the referral process Track referrals and follow-ups to ensure patients receive recommended care Obtain and document information from outside providers following patient appointments Collaborate with nurses, social workers, providers, and other members of the care team Maintain accurate and timely documentation within the electronic medical record Identify barriers that may prevent patients from completing referrals and assist with finding solutions Provide coverage and support for other care coordination functions as needed Maintain HIPAA compliance and confidentiality when handling protected health information Required Qualifications High school diploma or equivalent Minimum of 2 years of experience in referral management, healthcare coordination, medical administration, or a related healthcare setting Experience coordinating or scheduling patient referrals is highly preferred Strong understanding of healthcare referral processes Basic knowledge of medical terminology, CPT, and ICD-10 codes Strong communication and customer service skills Excellent organizational skills and attention to detail Ability to manage multiple patients, referrals, and competing priorities Comfortable working in a primarily phone-based role Preferred Qualifications Experience working with a senior or geriatric patient population Experience working in a primary care or specialty clinic Experience coordinating referrals to outside specialists Experience communicating with insurance carriers and provider offices Experience with electronic medical records, particularly eClinicalWorks or similar systems Experience with patient outreach and appointment scheduling Bilingual skills are a plus Schedule Monday–Friday 8:00 AM–5:00 PM What Our Client Offers Opportunity to support and positively impact a senior patient population Growing healthcare organization Collaborative care team environment Opportunity to make a meaningful difference by helping patients access necessary specialty care #J-18808-Ljbffr
