Patient Care Coordinator

Providing exceptional support for patients and providers, the full-time Patient Care Coordinator will manage incoming inquiries, schedule appointments, and handle administrative tasks in a remote setting. Key responsibilities Answer incoming patient inquiries and schedule appointments while making outbound calls to patients, pharmacies, and insurance companies Create and manage tickets in ServiceNow, ensuring effective communication and scheduling for providers Support clinicians through Microsoft Teams Chat, facilitating patient follow-ups and managing referral information Required qualifications Experience in phone, email, and chat-based customer service Ability to multitask effectively in a fast-paced environment Experience scheduling appointments and managing administrative tasks Medical Reception experience and familiarity with Electronic Health Records (EHR) is a plus Proficiency in Microsoft Office (M365) and experience with Microsoft Teams
