We’re seeking an organized, personable team member to support office operations and build community referral relationships.

Responsibilities:

• Handle communications, records, documents, and referral coordination.
• Build relationships with hospitals, skilled nursing facilities, assisted living communities, physician offices, and other referral sources.
• Conduct outreach visits, track follow-ups, and relay referral information to intake staff.
• Protect client privacy and patient choice, and complete required orientation and compliance training.

Qualifications: Strong communication, organization, and computer skills. Ability to travel locally. Administrative or healthcare outreach experience preferred.

Nonclinical employee role: No clinical advice or care, or monitoring, supervision, management, or training of referred independent-contractor caregivers. Report concerns to registry management.
