Position Details:

• Primary responsibilities include case management, care coordination, and client advocacy
• New Jersey Social Work license (LSW/LCSW) preferred; qualified candidates with relevant experience are encouraged to apply

Key Responsibilities:

• Provide comprehensive case management services to individuals and families
• Conduct client assessments and develop individualized service plans
• Coordinate care and connect clients with appropriate community resources and support services
• Collaborate with healthcare providers, community agencies, families, and multidisciplinary teams
• Maintain accurate, timely documentation and case records
• Monitor client progress and advocate for access to appropriate services and resources
• Ensure services are delivered in accordance with professional standards and agency policies
