Registered Nurse - Hospice

Job Description

Job Summary:
Seeking a compassionate Registered Nurse with hospice experience of at least 1 year to provide in-home and facility-based hospice care across multiple counties. The role involves delivering quality patient care while working collaboratively with another RN in a dynamic outpatient and inpatient setting.

Responsibilities:

• Administer hospice care to terminally ill patients in their homes and nursing facilities.
• Assess patient needs, develop care plans, and coordinate services according to individualized requirements.
• Document patient progress accurately using the EMR system, specifically EPIC.
• Collaborate with a team of RNs to ensure consistent and comprehensive patient care delivery.
• Follow facility and regulatory protocols to maintain compliance and quality standards.
• Educate patients and families on disease management, symptom control, and end-of-life planning.

Qualifications:

• Current WV RN license required.
• Minimum of 1 year hospice experience preferred.
• Hold BLS certification.
• Demonstrate strong assessment, communication, and caregiving skills.
• Able to work independently and as part of a team in a fast-paced environment.
• Familiarity with Epic EMR system is a plus.
