Job Title: Enhanced Care Management - Care Manager/Care Coordinator Location: Hybrid (Must reside in one of the following counties: Monterey County, San Benito County) Organization: Cecilia Holistic & Wellness Center Employment Type: Full-Time About Us: Cecilia Holistic & Wellness Center is a community-rooted organization committed to bridging the gap in care by providing wraparound support services that promote wellness, stability, and self-sufficiency. We serve individuals and families with compassion, respect, and a holistic approach to care. Position Overview: We are looking for a dedicated, organized, and tech-savvy Care Manager to join our remote team. This position requires a deep commitment to community service and holistic wellness. While the role is hybrid, candidates must reside in the servicing county and be available for local travel and driving for client visits, assessments, or community events.
