With compassion, professionalism, and purpose, we support every step of our participants’ journeys toward independence and well-being.**Position Summary**The **IDD Program Specialist** plays a pivotal role in supporting individuals with Intellectual and Developmental Disabilities (IDD) by coordinating, monitoring, and evaluating the services they receive. This role involves working closely with individuals, families, supports coordinators, and community resources to ensure that individuals with IDD have access to appropriate supports that align with their needs and goals.The IDD Program Specialist helps implement person-centered care, monitors service delivery, ensures compliance with regulations, and advocates for individuals’ rights and needs.**Key Responsibilities****1. Individual Care and Service Coordination**- **Assessment and Care Planning:** Conduct individual assessments to determine needs for HCBS services (personal care, nursing care, respite care, home modifications, etc.) and develop person-centered care plans.- **Service Coordination:** Facilitate delivery of appropriate services (personal care aides, homemaker services, skilled nursing) to ensure individuals receive timely and adequate care, collaborating with individuals and families. This includes onboarding documentation, scheduling, meet-and-greets, and ongoing monitoring.- **Community Outreach and Networking:** Conduct outreach to supports coordinators, Administrative Entities, and community organizations to inform them of available services, maintain relationships, and identify potential individuals for services.- **Referral Generation:** Drive business by generating referrals from community partners for individuals with IDD needing services and supports. **2. Referrals, Staffing Coordination, and Onboarding**- **Assess Staffing Needs:** Collaborate with regional managers and team leads to evaluate staffing requirements for current and upcoming referral needs.- **Referral Process Management:** Ensure staffing referrals are timely, accurate, and aligned with individual needs.- **Candidate Evaluation:** Review resumes, conduct preliminary interviews, and assess candidate qualifications and fit.- **Facilitate Interviews:** Coordinate interviews and verify candidate suitability.- **Onboarding Support:** Manage onboarding to ensure completion of documents, background checks, and certifications.- **Training Needs Assessment:** Identify training or certification needs for new hires and coordinate sessions to ensure readiness. **3. Service Coordination and Case Management**- **Implement Individual Service Plans (ISPs):** Collaborate with individuals, families, and supports coordinators to implement and monitor ISPs aligned with a person-centered approach.- **Monitor Service Delivery:** Track and review services to ensure individual needs are met as outlined in ISPs.- **Ongoing Assessment:** Continuously assess and update services based on changes in health, behavior, or needs. **4. Compliance and Documentation**- **Monitoring & Oversight of Direct Support Staff:** Ensure DSPs submit daily service notes aligned with participant needs, including accurate time recording and GPS locations, and approve notes for review.- **Ensure Regulatory Compliance:** Verify services comply with state and federal laws, Medicaid requirements, and local developmental disability guidelines. Maintain active participant and DSP profiles with proper documentation and certifications.- **Maintain Documentation:** Accurately document and maintain ISPs, assessments, service notes, and incident reports.- **Incident Reporting:** Ensure proper documentation and reporting of incidents involving individuals with IDD. **5. Advocacy and Individual Support**- **Individual Advocacy:** Advocate for the rights and needs of individuals with IDD, supporting service choices, daily life, and community participation.- **Promote Independence:** Assist individuals in gaining independence in daily living, employment, and community involvement. **6. Training and Staff Supervision**- **Staff Training:** Provide training and guidance to direct support staff, family members, and team members on best practices, person-centered planning, and internal policies.- **Supervision:** Oversee DSPs to ensure adherence to service plans, safety standards, quality support, and accurate documentation. **Qualifications** **Education**- Bachelor’s degree in Social Work, Psychology, Special Education , Human Services, or a related field preferred. **Experience:**- Minimum of 2 years working with individuals with intellectual and developmental disabilities, especially in case management, program coordination, or service delivery.- Experience with person-centered planning, behavioral interventions, and community integration highly preferred.- Familiarity with IDD service systems, including Medicaid, waiver programs, and state/federal regulations. **Clearance Requirements:**- BCI (required) and FBI (if resident of Ohio for less than 5 years).- Motor Vehicle Check.- Authorization to work in the United States.- Additional exclusionary checks as applicable. **Reports to:** IDD Team Lead **Competency Requirements**- Strong communication and interpersonal skills for working with individuals, families, and professionals.- Knowledge of IDD-related resources and support services.- Problem-solving and crisis management skills .- Ability to handle sensitive situations with empathy and professionalism.- Autonomy and independence.- Case management skills .- Cultural competence.- Adaptability to change. **Work Environment**- Office-based, **Monday–Friday, 8:30 AM–5:00 PM**.- May require travel to individuals’ homes, community settings, or healthcare facilities. **Legal Disclaimer** This job description reflects the general nature and scope of the role and is not exhaustive.
