Program Consultant
Lutheran Social Services
Lutheran Social Services seeks an IRIS Program Consultant to support adults in self-directing their long-term care services. In this role, you will provide person-centered consulting, assess needs and goals, and develop individualized service and support plans that promote independence and community living. You will educate participants on IRIS options and budgets, coordinate services with providers and families, and monitor health, safety, and satisfaction through home visits and ongoing contact. Working within a mission-driven, faith-based nonprofit, you’ll document cases, ensure regulatory compliance, and collaborate with interdisciplinary teams to advance dignity, choice, and social justice for those you serve.
