Per Diem Registered Nurse (RN) – Hospice After-Hours Care (Per Diem) Saratoga RegionThe Community Hospice is seeking a compassionate and dedicated per diem Registered Nurse (RN) to join our After-Hours Hospice Care Team. From comprehensive training to providing necessary protective equipment, we're committed to creating a secure environment for you as you deliver exceptional care in our clients' homes.What you will do:As an After-Hours Hospice RN, you will respond to patient and family needs during overnight hours, providing skilled nursing assessments, symptom management, and supportive care in homes and nursing facilities. You will collaborate closely with the hospice care team to ensure continuity of care and promote comfort, dignity, and quality of life.Responsibilities:Provide after-hours nursing visits to hospice patients in home and facility settingsAssess and manage symptoms, addressing urgent patient and family needs.Provide education, guidance, and emotional support to patients and caregivers.Coordinate care with the interdisciplinary hospice team.Communicate changes in patient condition and ensure appropriate follow-up.Maintain accurate and timely clinical documentation.What you will need:Valid NYS RN license required.1-2 years previous RN experience required, homecare and/or Hospice experience preferred.Ability to learn/use an electronic medical recordHospice experience is preferred but not required. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
