Your work sets the tone—providing comfort, clarity, and reassurance while guiding patients and families through the transition to hospice care.What You'll DoServe as a key liaison between patients, families, providers, and our interdisciplinary teamReview and coordinate referrals to determine hospice eligibility in collaboration with physicians and care teamsMeet patients where they are—at home or in care facilities—to complete initial assessmentsInitiate care plans, including ordering medications, medical equipment, and supportive servicesPartner with social workers, chaplains, aides, therapists, and volunteers to provide holistic careEnsure a smooth, supportive handoff to the Hospice Case ManagerWhere You'll WorkThis role offers a dynamic schedule, combining:Time in our central intake office in AlbanyField visits to patients in homes, nursing homes, and assisted living communities across the regionWhat You BringActive New York State RN licenseASN required; BSN preferredHome care experience (1+ year) strongly preferredHospice or end-of-life care experience is a plusValid driver's license and reliable transportationStrong communication skills—you connect easily with patients, families, and clinical teamsClear, concise documentation abilitiesComfort with computers and electronic health recordsExcellent organization and time management skillsWhy Join UsAt The Community Hospice, you'll be part of a mission-driven organization that puts compassion at the center of everything we do. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
