Dayshift LPN (Licensed Practical Nurse) Full Time or Part Time Location: Cincinnati, Ohio Company: Meadowbrook Care Center Job Summary: We are seeking a compassionate and dedicated Dayshift LPN to join our team at Meadowbrook Care Center. As a Dayshift LPN, you will play a vital role in providing high-quality care to our residents, while working collaboratively with our interdisciplinary team to ensure exceptional outcomes. Responsibilities Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians Conduct vital signs, monitor resident conditions, and report changes to the healthcare team Develop and implement individualized care plans, in conjunction with the interdisciplinary team Collaborate with rehabilitation staff to develop and implement treatment plans Participate in quality improvement initiatives to ensure optimal resident outcomes Maintain accurate and timely documentation of resident care and services Assist with admissions, transfers, and discharges as needed Perform other duties as assigned by the RN or Nurse Manager Requirements Current LPN licensure in the state of Ohio 0 years of experience (new graduates welcome to apply!)
