You’ll design and implement individualized therapy plans to help patients regain independence in their home environment while coordinating care with the broader rehabilitation team.Key Responsibilities:Provide effective patient care using approved therapy techniques and proceduresEvaluate patients’ condition, function level, and treatment goalsDevelop individualized treatment plans and update as neededSupervise COTAs and HHAs per care plan requirementsDocument treatment notes and progress in the patient’s chart dailyAssist patients and families with home exercise programs and adaptive equipment needsRecommend and evaluate adaptive equipment for the home settingMaintain compliance with HIPAA regulations and company policiesCoordinate total patient care with the healthcare teamComplete discharge summaries and obtain orders when neededSubmit patient documentation and visit records in a timely mannerQualifications:Licensed Occupational Therapist (OT) in the State of CaliforniaMinimum of 1 year of OT experience preferredExperience with OASIS and WellSky (Kinnser) strongly preferredCurrent physical (within the last 6 months)Current TB test (within last 12 months) and CPR certificationValid driver’s license and reliable transportationAbility to attend a 2–3 day orientation at our San Diego officeApply Today!Join Aspire Home Healthcare and bring your skills and compassion into the homes of those who need it most.
