Care Manager II - Health Home (Chautauqua County)

Job Overview Elm & Oak Health is hiring Care Managers in the Chautauqua County Area. This full‑time position is based primarily from home with travel within the county for member visits. Salary & Benefits Grade 207: $46,948 – $57,380 annually. Comprehensive benefits package and non‑monetary perks. Elm & Oak Health is committed to equal pay opportunities for all employees. Essential Duties and Functions Care Management (50%) Receive and triage referrals for Health Home services. Contact referrals within appropriate timeframe; address urgent or emergent issues and schedule intake appointments. Conduct comprehensive biopsychosocial assessments for adults and/or children. Develop therapeutic relationships and person‑centered interventions. Coordinate services with identified healthcare providers and agencies. Create and share a Person‑Centered Plan of Care with members and providers. Disseminate care information to involved individuals as approved by the member. Interview referrals and families, collect data, and provide health education. Make recommendations for continuation or change in services. Maintain minimum monthly telephonic contact and in‑person visits at least every three months (varying by member acuity). Seek consultation for complex medical, behavioral health, or psychosocial needs. Demonstrate cultural responsiveness when working with members and the community. Travel for home visits and community activities. Adhere to Elm & Oak Health professional boundaries and protocols. Documentation (30%) Complete all required documentation promptly, clearly, and concisely. Demonstrate aptitude in program software within 3 months of training. Complete all required assessments, including a comprehensive assessment, Health Home authorization, HML assessment, and other regulatory documents. Document a Person‑Centered Care Plan with client and provider collaboration. Review and update assessments as required by regulations. Maintain thorough, actionable documentation at least once per month. Use the electronic record for documentation of care management, coaching, and termination. Collaboration (15%) Participate as a member of the multidisciplinary Care Management team. Initiate and facilitate member‑focused meetings with providers and significant others. Participate in inter‑agency teams to enhance services for members. Collaborate with Elm & Oak Health staff, providers, and Monroe Plan staff to coordinate care. Manage conflict to support positive outcomes. Participate in community activities to promote health and public awareness. Assist in locating members in the community through home visits. Attend and participate in in‑service training. Communication (10%) Present professionally to develop therapeutic relationships with members and providers. Provide feedback to providers on progress and barriers. Demonstrate listening skills to support member engagement. Offer program information to members, providers, and other organizations. Other Functions and Responsibilities Cannot perform tasks governed by a license or registration (e.g., diagnoses, medication recommendations). Cannot transport active Monroe Plan members. Cannot perform hands‑on care. Position Limitations Travel required within Chautauqua County Area. Must possess a NYS driver’s license and reliable transportation. Minimum Requirements, Licenses, and Certifications Master’s degree in a specified field plus 1 year of direct services experience for individuals with serious mental illness, developmental disabilities, alcoholism/substance abuse, or children with SED. Bachelor’s degree in a specified field plus 2 years of direct services experience for the same populations. Credentialed Alcoholism & Substance Abuse Counselor (CASAC) plus 2 years of direct services experience. Bachelor’s or higher in any field plus 3 years of experience establishing community services for the same populations, or 2 years as a Health Home Care Manager serving the SMI or SED population. Ability to respect individual/family diversity and maintain confidentiality. Team‑player skills and collaborative ability with providers and community services. Excellent oral and written communication skills. Ability to work independently and manage time appropriately. Strong organizational skills. Computer literacy. NYC license and reliable transportation for daily member visits. Preferred Qualifications Previous experience as a Health Home Care Manager.
