Case Management Discharge Planning Assistant

$23.74 - $31.94 per hour

The Discharge Planning Assistant is an active member of the care transitions team, working collaboratively with the Inpatient RN Case Managers to provide direct administrative support to the discharge planning process. This role spends time on clinical units and directly interacts with patients and their families, clinical and ancillary hospital staff, and other internal and external customers. Duties / Responsibilities Supports the care transitions discharge functions by entering referrals as directed. Supports the gathering of clinical information for payor authorizations. Provides required documentation to third‑party vendors to secure post‑acute transition services. Provides direct support to the RN Case Manager in the clinical units. Verifies patient demographics, including address, telephone number, and insurance. Verifies the presence of a health care proxy. Distributes regulatory notices and completes all required documentation. Assists with securing patient resources as requested. Assists with the post‑acute referral process and monitors status, following up on screening determinations as necessary. Identifies in‑network providers such as VNA, DME vendors, rehabilitation facilities, etc. Collaborates with the RN Case Manager to facilitate timely discharge to the next level of care. Demonstrates thorough knowledge of various computer/information systems, including Careport, EMR, and Outlook. Collects/inputs data or information in appropriate databases as directed by Care Transitions leadership. Provides documentation to patients, families, or caregivers under the direction of RN Case Managers. Independently identifies and communicates any problems or issues that affect departmental goals and outcomes. Performs routine clerical tasks such as opening mail and providing accurate information to callers. Education Required: High School Diploma or GED. Preferred: Associate’s Degree or Bachelor’s Degree. Experience and Qualifications Excellent computer skills, including proficiency with Meditech, Epic, Cerner, and MS Outlook. Strong communication, interpersonal, and customer‑service skills. Excellent assessment and problem‑solving skills. Knowledge and understanding of medical terminology (preferred). A minimum of 2 years of experience in healthcare (preferred). Knowledge of post‑acute community resources (preferred). Strong organizational and prioritizing skills (preferred). Certification BLS (Basic Life Support). Pay Range Base hourly wage: $23.74 - $31.94. Actual compensation may exceed this range depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices.
