About the Role: As an MDS Coordinator, you will be responsible for: Collecting and analyzing data to develop a comprehensive care plan for each patient Coordinating with care providers, therapists, and other healthcare professionals to ensure seamless care delivery Assisting with the development and implementation of individualized treatment plans Monitoring patient progress and making adjustments to care plans as necessary Maintaining accurate and timely records of patient care and treatment Responsibilities: Collaborate with interdisciplinary teams to develop and implement comprehensive care plans Gather and analyze data to identify trends and areas for improvement in patient care Develop and maintain accurate and timely records of patient care and treatment Assist with the implementation of individualized treatment plans and monitor patient progress Participate in quality improvement initiatives to enhance patient care and outcomes Maintain effective communication with patients, families, and healthcare team members Requirements: Current certification as a Licensed Practitioner (LPT) or Registered Nurse (RN) in the state of Florida Minimum of 1 year experience in a long-term care setting, as an MDS Coordinator Strong analytical and problem-solving skills Excellent communication and interpersonal skills Ability to work effectively in a fast-paced environment Familiarity with MDS software and electronic medical records What We Offer: Competitive salary Benefits package Opportunities for professional growth and development Collaborative and supportive work environment A chance to make a difference in the lives of our patients and residents #J-18808-Ljbffr Miramar Post-Acute Care Solutions
