Longevity Health Plan in Chicago, IL seeks a Licensed Account Manager to support SNF partners and drive ISNP enrollment goals across the territory. This field-based role reports to the Market VP and coordinates with Network, Clinical, and Project Management teams to deliver high-quality, compliant care. You will manage facility relationships, present enrollment strategies, ensure contracts and provider readiness, and travel within the assigned markets to achieve monthly targets while upholding #J-18808-Ljbffr Longevity Health Plan
