Physical Therapist

Position: Physical Therapist (PT) Role: Evaluate residents, develop personalized treatment plans, and provide skilled therapy to enhance mobility, strength, balance, safety, and independence in a resident-centered skilled nursing environment. Responsibilities Conduct comprehensive physical therapy evaluations. Create and implement individualized treatment plans based on resident needs and physician directives. Deliver therapeutic interventions to improve functional abilities. Assess and recommend assistive devices, mobility aids, and safety modifications. Monitor progress and adjust treatments accordingly. Educate residents, families, and caregivers on treatment and discharge planning. Maintain accurate documentation and participate in interdisciplinary care planning. Ensure compliance with all regulatory, safety, and professional standards. Qualifications & Skills Current Colorado PT license in good standing. Strong clinical assessment and treatment planning skills. Excellent communication and interpersonal abilities. Ability to work independently and collaboratively within a team. Experience in skilled nursing, rehabilitation, or long-term care preferred. Knowledge of Medicare and managed care rehabilitation services is a plus.
