Sapphire Nursing at Meadow Hill skilled Nursing Facility located in Orange County, New York is looking for a talented and hard-working Clinical Care Coordinator . The Clinical Care Coordinator at our facility will be responsible for managing patient care plans, coordinating health care services, and ensuring positive patient outcomes.

Key Responsibilities:

• Patient Care Management: Monitor patient progress and compliance with treatment plans. Develop and implement individualized care plans  based on patient assessments and healthcare provider recommendations.
• Coordination of Services : Collaborate with healthcare professionals, including doctors, nurses, and social workers to ensure comprehensive care delivery, schedule treatments, procedures, and follow up appointments.
• Patient Advocacy : Serve as a liaison between patients and healthcare providers, ensuring that patients understand their treatment options and resources available to them.
• Documentation : Maintain accurate records of patient interactions, treatment plans and progress notes to ensure continuity  of care and compliance with healthcare regulations.
• Education and Support: Education patients and their families about health management, treatment options, and available community resources. Provide emotional support and guidance throughout the treatment process.
• Required Qualifications:

Education : NYS RN or Licensed Social Worker (LSW) certification.

Experience : Two years experience in a skilled nursing facilities with care coordination or case management is highly desirable.

• Communication Skills : Strong verbal and written communication skills  are essential for interacting with patients, family and healthcare teams.
• Organizational Skills : Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
• Critical Thinking :  Strong problem solving skills to assess patient needs and develop appropriate care plans.
• Cultural Competency : Understanding and respecting diverse backgrounds and needs of patients to provide equitable care.
