Description

• Conduct evaluations for patients in both inpatient and outpatient settings and develop personalized treatment plans
• Deliver physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Collaborate with nursing staff, physicians, case managers, and fellow rehab professionals to enhance patient outcomes
• Guide patients and their families through therapeutic goals, discharge planning, and at-home exercise regimens
• Maintain detailed documentation of evaluations, treatments, and patient progress in alignment with facility and regulatory standards
• Engage in care team rounds and interdisciplinary meetings to ensure unified treatment approaches

Education

• Completion of a Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., in neurology, geriatrics) are advantageous but not required

Skills

• Previous experience is highly desirable
• Capability to treat patients facing complex medical and mobility challenges
