Description

• Conduct assessments of patients in both inpatient and outpatient environments and craft tailored treatment plans

• Inform patients and their families about therapeutic objectives, discharge processes, and home exercise regimens
• Maintain documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards
• Engage in care team meetings and interdisciplinary discussions to ensure seamless care transitions

Education

• Doctorate in Physical Therapy (DPT) or a comparable qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory
