Currently, this position is not eligible for visa sponsorship or STEM OPT extensions. Core Responsibilities: The Maine CDC Cardiovascular Health Program is looking for a Comprehensive Health Planner II to implement Cardiovascular grant strategies and lead efforts to improve the quality of Cardiovascular care in Maine. The Cardiovascular Health Coordinator manages the cardiovascular health-related work of the Program’s U.S. CDC cooperative agreements and works as part of a team to execute work plan activities. This position works with health care systems to facilitate quality improvement practices that improve health system workflows and patient outcomes, improve use of health information technology and clinical quality measurement, improve clinical workflows for the identification of undiagnosed hypertension, and to promote referrals to and participation in evidence-based programs such as Self-Measured Blood Pressure (SMBP). This position collaborates with stakeholders, contractors, advisory boards, and other Maine CDC/DHHS programs to develop, implement, and expand a comprehensive approach to chronic disease prevention in Maine You will also: • Serve as the primary contact for internal and external stakeholders and customers regarding cardiovascular health strategies/activities. • Work with the Program Manager and Diabetes Program, to monitor and implement pertinent federal grant work plans and budgets, including practice facilitation and quality improvement coaching with health care systems. Provides content for annual work plan and progress reports in federal grant reporting tool within established timelines. • Provide subject matter expertise on cardiovascular health promotion and the prevention of cardiovascular diseases and program representation, leadership, and expertise on all relevant committees and projects • Provide technical assistance to health care providers and evidence-based program providers to ensure effective cardiovascular disease prevention and self-management program facilitation. • Review and maintain up-to-date knowledge of cardiovascular disease prevention, self-management, and treatment best and emerging practices. • Assist the Chronic Disease Prevention Program in implementing a comprehensive, collaborative approach to addressing chronic diseases in Maine. • Contribute to strategic conversations, when requested and as experience and expertise dictates, on: • Application for and retention of funding opportunities • Legislative monitoring • Strategic Planning • Attend regularly scheduled meetings with Program staff, Project Officers, Contractors, Stakeholders, Partners and monthly meetings of the Chronic Disease Prevention and Control Program team. • Practice good customer service skills and behaviors in all contacts with clients, co-workers, and the public. Minimum Qualifications: A Bachelor’s Degree in Public/Community Health, Health Administration, Health Sciences or Health Education; Public Administration, Government, Social Work, or a closely related field and three (3) years professional experience in the planning, development, coordination and/or evaluation of health programs or services -OR- an equivalent seven (7) year combination of education, training, and experience. Preferred candidates will also have: • Knowledge of and experience with implementing systems change, Practice Facilitation, Quality Improvement, and adapting clinical workflows. • Knowledge of and experience with evidence-based cardiovascular health promotion programs and cardiovascular disease self-management programs. • Knowledge of and experience with U.S. CDC grant management requirements. • Experience convening and collaborating with various stakeholders, in both public and private sectors.
