This opportunity offers a 100% outpatient setting, strong clinical support, and the ability to build a patient panel within a mission-driven, community-focused health system. Practice Highlights · Established, high-demand primary care practice · Immediate patient volume available · 100% outpatient - no inpatient responsibilities · Team-based care model (PA, MA, split nurse) · Integrated Epic EMR · Strong referral network within Trinity Health Mid-Atlantic Qualifications · MD or DO from an accredited institution · Board Certified or Board Eligible in Family Medicine · Eligible for Pennsylvania licensure · Commitment to high-quality, patient-centered care Role Responsibilities · Provide comprehensive primary care for patients across the lifespan · Diagnose and manage acute and chronic conditions · Emphasize preventive care and population health management · Collaborate with interdisciplinary teams to optimize patient outcomes · Maintain timely and accurate documentation · Participate in quality and performance improvement initiatives Schedule & Call · Full-time outpatient schedule (34 patient-facing hours) .
