Responsibilities

• Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations.
• Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability.
• Deliver direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and the use of modalities as ordered.
• Monitor progress and adjust care plans to ensure goal achievement; participate in discharge planning as appropriate.
• Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
• Supervise and guide Physical Therapist Assistants and therapy aides in accordance with licensure requirements.
• Accurately document treatment sessions, progress notes, and billing information in a timely manner.
• Ensure compliance with state, federal, and facility documentation and practice standards; participate in quality improvement initiatives as part of the rehab team.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice (Missouri).
• Experience in skilled nursing facility (SNF) or geriatric rehabilitation preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently and as part of an interdisciplinary team.
• Eligibility to practice in a skilled nursing/long-term care setting in Missouri.
