Job Overview
Radiant Nursing and Rehab at Palatka , in Palatka, FL a dedicated and adaptable Physical Therapist to provide exceptional patient care on a PRN (as-needed) basis. The ideal candidate will possess a strong foundation in physiotherapy practices, with experience across various clinical settings including inpatient, outpatient, home health, and hospital environments. This role offers the opportunity to work flexibly while utilizing comprehensive skills in patient assessment, treatment planning, and rehabilitation. The successful candidate will demonstrate expertise in discharge planning, care plan development, and a broad understanding of physiology and anatomy to optimize patient outcomes.

Pay begins at $50/hr and increases with experience

Duties

• Conduct thorough patient evaluations to determine functional limitations and develop individualized care plans.
• Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, and geriatrics interventions to promote recovery.
• Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions across care settings.
• Document patient progress accurately within electronic medical record (EMR) systems, maintaining compliance with healthcare regulations.
• Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing recovery.
• Manage acute care cases by providing timely interventions in hospital or inpatient settings.
• Stay current with advancements in physiotherapy techniques and medical terminology to enhance patient care quality.

Requirements

• Valid licensure as a licensed Physical Therapist with relevant state or regional credentials.
• Proven experience in inpatient, outpatient, hospital, or home health settings with a focus on patient care.
• Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice.
• Demonstrated ability to develop comprehensive care plans tailored to individual patient needs.
• Experience with EMR systems for documentation and treatment tracking purposes.
• Familiarity with discharge planning processes and community-based rehabilitation programs.
