What You'll Do

• Provide skilled nursing care to patients in their homes, assessing health needs and carrying out care plans
• Educate patients and families on health management and disease prevention
• Collaborate with healthcare teams and community resources to ensure coordinated care
• Monitor patient responses to treatments and document findings accurately
• Support with health assessments, medication management, and clinical interventions
• Follow established guidelines, protocols, and policies, ensuring compliance and safety
• Document clinical activities and maintain confidential records

What You Need

• Valid Massachusetts RN license (or eligible to obtain)
• 2+ years of clinical experience in community health, home health, or related field
• BLS certification and DEA license
• Ability to work independently and exercise sound judgment
• Strong communication and interpersonal skills
• Valid driver's license and ability to drive in various weather conditions

• Experience with electronic medical records (EMR) systems
• Previous home health or community health experience
• Ability to be flexible with work hours and on-call responsibilities
