Urgent Care Physician Assistant

We are seeking a dedicatedNurse Practitioner or Physician Assistant to join our growing Post-Acute Careteam Wakefield, RI. This full-time position provides an excellent opportunity todeliver high-quality, patient-centered care to individuals transitioning fromacute hospital settings to skilled nursing and rehabilitation facilities.
Additional Openings: Wakefield,Rhode Island and North Conway, New Hampshire. Providers will workcollaboratively with interdisciplinary teams to manage chronic and acutemedical conditions, support recovery, reduce hospital readmissions, andoptimize patient outcomes in the post-acute setting.
Position Highlights:
- Full-time schedule: Monday -Friday, 9:00 AM - 5:00 PM
- No weekends
- No call responsibilities
- Facility-based rounding inpost-acute care settings
- Autonomy with strongclinical and operational support
- Opportunity to buildlong-term relationships with patients and care teams
Compensation & Benefits:
- Base salary: $120,000annually
- Total earning potential:$180,000 - $220,000+

Requirements:
- Graduate of an accreditedNurse Practitioner or Physician Assistant program
- Current and unrestricted NPor PA license in the applicable state or eligibility for licensure
- National board certificationas a Nurse Practitioner or Physician Assistant
- DEA registration oreligibility to obtain
- Minimum of one year ofclinical experience preferred; experience in post-acute care, skilled nursing,geriatrics, internal medicine, family medicine, hospital medicine, or long-termcare strongly preferred
- Strong clinical assessment,diagnostic, and care coordination skills
- Excellent communication andinterpersonal abilities
- Ability to workindependently while collaborating effectively within a multidisciplinary team
- Proficiency with EMR systems
