The successful candidate will benefit from a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.

Work Environment

As a Physical Therapist at Stretch Synaptic Therapy Services, you will be working in a home care environment, directly within the comfort of our clients' homes. Key Responsibilities

• Plan and implement personalized physical therapy treatments aimed at preventing disability and alleviating pain, following each client's unique plan of care.
• Oversee and provide guidance to physical therapy assistants, ensuring the effective execution of treatment protocols and enhancing client recovery outcomes.
• Conduct thorough evaluations of patients' functional abilities, employing standardized tests for muscles, nerves, joints, and overall function.
• Assess and adapt home environments to facilitate rehabilitation and enhance client independence through informed recommendations.
• Deliver prescribed physical therapy services in clients' homes, including manual therapeutic techniques and instruction in exercise programs tailored to individual capabilities.
• Administer modalities such as traction, therapeutic massage, and other physical agents as part of the treatment regimen.
• Evaluate, fit, and modify assistive devices (prosthetics and orthotics) in collaboration with the medical team to optimize patient mobility and function.
• Collaborate in the development and ongoing review of care plans, ensuring that they meet changing client needs and maximize therapeutic outcomes.
• Document clinical records meticulously, including evaluations, progress notes, and discharge summaries, and ensure timely submission in accordance with agency policies.
• Participate actively in interdisciplinary team discussions, contributing to comprehensive planning and evaluating treatment protocols in case conferences.
• Provide education and support to clients and their families regarding therapy practices, equipment use, and rehabilitation goals, fostering a collaborative approach to care.
• Supervise and mentor Physical Therapy Assistants, ensuring compliance with state regulations in their treatment practices.
• Conduct on-site functional assessments and adjust care plans as necessary, leveraging community resources for enhanced patient support.
• Complete additional assignments that align with the company's objectives and enhance team performance.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.

• In-depth knowledge of physical therapy techniques and a commitment to continuing education in the field.
