Licensed Practical Nurse
San Luis Care Center
San Luis Care Center in San Luis, CO is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care in our skilled nursing and rehabilitation facility. The LPN will deliver direct bedside care, administer medications and treatments, monitor vital signs and resident conditions, and document accurately in medical records. The role collaborates closely with RNs, therapists, and CNAs to support care plans, rehabilitation goals, and safe transitions. This position offers the chance to build strong relationships in a close-knit, rural community while contributing to a supportive, team-focused, family-like environment.
