Key Responsibilities\n

• Conduct thorough patient intakes focusing on diagnostic and functional evaluations, providing accurate recommendations to primary care providers.

• Deliver brief, focused therapeutic interventions to individuals experiencing mental health challenges, aligning with established care plans and goals.

• Provide expert guidance to primary care providers on managing mental health issues within the clinic, assisting them in determining suitable referrals for specialty care.

• Initiate follow-up communication with patients to monitor progress and determine necessary adjustments to treatment plans.

• Develop and implement comprehensive relapse prevention plans to ensure patient stability and ongoing success in coping with their conditions.

• Identify 'at risk' patients and collaborate with interdisciplinary teams to create preventive strategies and enhance overall health outcomes.

• Maintain meticulous clinical documentation, including patient progress and treatment strategies, while ensuring compliance with agency and Federally Qualified Health Center (FQHC) guidelines.

• Educate patients on self-management strategies, coping mechanisms, and lifestyle adjustments that promote better mental and physical health.

• Engage in interdepartmental collaboration, providing consultation to clinic management and other team members regarding behavioral health services and outcome evaluations.

Patient Population / Client Focus\n

You will work with a broad spectrum of patients, including individuals experiencing anxiety, depression, and other complex mental health conditions. Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling or a related field.

• Current and active Licensed Professional Counselor (LPC) license in New Jersey.

• Understanding of developmental psychopathology and family systems therapy principles.

• Ability to develop strong therapeutic alliances with patients and maintain effective communication with multidisciplinary teams.

Preferred Experience\n

• Fluency in both English and Spanish is highly desired but not mandatory.

• Previous experience in a community health setting preferred, with a solid understanding of various community resources.

Compensation and Benefits\n

The compensation for this position ranges from USD 87,000 to 105,000 annually, commensurate with experience and qualifications.
