Responsibilities:

• Provide personalized home health care services to patients, including assistance with ADLs, medication management, and other essential care activities

• Develop and maintain strong relationships with patients, families, and healthcare teams to ensure seamless care transitions and coordination

• Monitor patient vital signs, report any changes or concerns to the healthcare team, and implement care plans as directed

• Maintain accurate and detailed records of patient care, including noting progress, outcomes, and any changes or challenges

• Collaborate with other healthcare professionals, including nurses, therapists, and social workers, to ensure comprehensive care

• Demonstrate a commitment to excellence in patient care, following all necessary protocols, policies, and procedures

• Stay up-to-date with industry developments and best practices to continuously improve patient outcomes and care delivery

Requirements:

• 1 year of experience in a healthcare setting

• High School diploma or equivalent

• Certification as a Home Health Aide (CHHA)
