Conduct comprehensive evaluations of patients in inpatient or outpatient facilities and curate personalized treatment plans. Deliver targeted physical therapy interventions concentrating on improving mobility, strength, balance, and overall function. Partner with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimized patient outcomes. Educate patients and their families about therapeutic goals, discharge planning, and at-home exercises. Maintain thorough documentation of evaluations, treatments, and patient progress, adhering to facility and regulatory protocols. Engage in care team rounds and interdisciplinary meetings to guarantee coordinated patient care.

A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution is mandatory. Valid state licensure as a Rehabilitation Therapist / Physical Therapist is required. CPR certification is preferred. Specialized certifications (e.g., in neurology or geriatrics) are advantageous but not mandatory.

Prior experience is highly preferred. Ability to treat patients with intricate medical and mobility challenges. Excellent communication and collaboration skills are essential. Proficient in utilizing digital charting and clinical documentation systems.
