Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Team. This position will involve providing direct care to clients within a 35-mile radius of Grand Rapids. Responsibilities Perform initial and comprehensive assessments and deliver compassionate care to clients Collaborate with physicians and other care team members to develop a Plan of Care Promote client and family/caregiver independence through counseling and education Identify barriers to desired outcomes and evaluate outcomes of care Prepare clinical documentation, progress notes in a timely manner using iPads for the EMR system Participate in on-call rotation Location Grand Rapids, covering a 35-mile radius around GR. Schedule / Shift Monday - Friday Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance On-call rotation every 6-8 weeks to cover PRN visits, on-call triage and weekend admissions Compensation Our compensation model includes bonuses for high-quality client outcomes and efficient case management Premium pay for weekend & holiday visits Qualifications Licensed RN in Michigan; BSN preferred Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred Passion for delivering high quality care to older adults Paid Time Off (PTO) Generous 403b match with access to a professional Financial Planner at no cost Why work for Atrio?
