Registered Nurse Case Manager – Nursing

Schedule: Monday to Friday, 08:00 AM - 05:00 PM

Assignment Details: Holiday coverage is not required. The traveler will start not less than 6 weeks after all necessary documentation is received.

Requirements

• Active Registered Nursing license in Alaska or other U.S. state
• Minimum of 1 year of case management experience in a clinic or hospital setting within the last 2 years
• Basic Life Support (BLS) certification
• Graduate of an accredited school of nursing; a bachelor's degree is preferred
• Case Management certification is preferred

Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care
• Administer medications and treatments safely per provider orders
• Provide patient and family education regarding care and discharge needs
• Collaborate with providers and interdisciplinary teams for care coordination
• Document assessments and interventions accurately in the electronic health record
• Participate in quality and performance improvement activities
