Registered Nurse - Ambulatory Care/Clinic/Outpatient

Job Summary:
The Ambulatory Registered Nurse (RN) Case Manager in Primary Care provides comprehensive outpatient nursing care to patients, supporting their health needs through assessment, education, and coordination of services. This role emphasizes patient-centered, culturally responsive care in a fast-paced clinic environment.

Responsibilities:

• Perform thorough patient assessments and develop individualized care plans.
• Administer medications and treatments safely according to provider orders and policies.
• Recognize clinical changes and initiate appropriate interventions or escalate care as needed.
• Collaborate with providers and interdisciplinary teams to coordinate patient care and facilitate referrals.
• Document assessments, interventions, and patient outcomes accurately in the electronic health record.
• Promote safety, infection control, and regulatory compliance while maintaining quality standards.

Qualifications:

• Hold an active, unrestricted Alaska Registered Nursing license or have applied for one prior to start date.
• Possess current BLS certification; Case Management certification preferred.
• Graduate from an accredited nursing program; Bachelor’s degree preferred.
• Demonstrate at least 2 years of recent case management experience in a clinic or hospital setting.
• Exhibit strong clinical judgment, critical thinking, and effective communication skills.
