Key Responsibilities\n

As a Licensed Clinical Social Worker, your primary responsibilities will include:

• Conducting comprehensive psychosocial assessments to diagnose and develop individualized treatment plans for patients in hospice care.

• Implementing evidence-based therapeutic interventions such as solution-focused therapy, problem-solving techniques, and motivational interviewing to enhance patient coping strategies.

• Monitoring clients’ progress toward therapeutic goals through regular evaluations and adjustments to treatment plans, ensuring efficacy and adherence to prescribed therapies.

• Collaborating with a multidisciplinary team, including primary care physicians and other mental health professionals, to coordinate holistic care delivery and address all facets of a patient’s health.

• Providing education and support to families to assist them in understanding the emotional aspects of hospice care.

• Maintaining accurate and Confidential electronic patient records, documenting treatment sessions and correspondence in accordance with regulatory and organizational policies.

Patient Population / Client Focus\n

This role is particularly focused on serving patients within a hospice setting, where empathy and understanding are paramount. You will work with individuals and families dealing with terminal illnesses, providing psychological support and counseling tailored to enhance their end-of-life experience. Qualifications\n

To be successful in this role, candidates must demonstrate the following qualifications:

• A Master’s degree in Social Work from an accredited institution.

• Current, unrestricted LCSW licensure to practice independently in Florida.

Compensation and Benefits\n
