Registered Nurse

The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that promotes healing and wellness. Nursing practices are based on evidence, current research, and established policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through supportive relationships among nursing staff and across other disciplines. The role includes coordinating and providing direct patient care activities, such as nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is essential. Providing patient and family-centered education is part of the role. The Registered Nurse will coordinate care and develop individualized treatment plans as applicable. A comprehensive assessment is performed and documented in accordance with the nursing scope of practice. Consideration is given to the patient's culture, age, and developmental level. Patient risk factors and presenting complaints are identified. Health information may be obtained from family or others as necessary. Both objective and subjective data are collected. Initial admission assessments are verified if completed by other healthcare team members. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions relevant to the patient's culture and age are designed. Desired outcomes are determined in coordination with the patient and family following the medical plan of care. Patient care therapies and procedures are implemented within the scope of practice according to established policies and professional guidelines. Procedures, treatments, and processes are explained to patients and families. Initiatives are taken to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. Tasks are delegated appropriately while overseeing team members' performance in patient care activities. The accuracy and completeness of delegated patient care are monitored to ensure effective implementation of the plan of care. Educational needs of patients related to prioritized problems are identified and addressed. The patient discharge process is facilitated by timely execution of discharge orders and coordination with Case Management. Input into patient/nurse assignments is provided, considering staffing, unit activity, and patient acuity. Active participation in unit-based activities to support organizational values and priorities is expected. A positive group dynamic and healing environment are promoted through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, and current research. Participation in departmental or unit quality improvement activities is encouraged. A holistic perspective on patient care is maintained throughout.
