Key Responsibilities Perform patient assessments and reassessments per policy and procedure or as warranted by patient condition, demonstrating the ability to make sound independent clinical decisions. Initiate, prioritize, and implement individualized plans of care; review and revise plans as indicated in collaboration with patients, families, and interdisciplinary teams. Monitor and document patient responses to clinical interventions in a timely and accurate manner. Facilitate, coordinate, and evaluate patient and family education specific to care, treatment, services, and discharge needs. Delegate tasks appropriately to licensed and non-licensed personnel; serve as a mentor and preceptor to staff while fostering teamwork. Participate in quality, safety, and service process improvement efforts; comply with regulatory standards and contribute to the Practice Governance model. Required Qualifications Active Registered Nurse (RN) licensure. Nursing Diploma or Associate's Degree in Nursing (ASN). Minimum of two (2) years of acute care RN experience.
