Key Responsibilities\n

In this role, you will be responsible for:

• Assisting primary care providers in recognizing and managing mental health and psychosocial issues as a key member of our care team.

• Conducting thorough patient intakes that focus on diagnostic and functional evaluations, followed by formulating recommendations for treatment goals and plans.

• Delivering brief, focused interventions tailored to patients requiring mental health services.
\n
• Consulting with primary care providers to determine which patients may benefit from in-house services versus referrals to specialized mental health facilities.

• Initiating follow-ups with patients to assess their progress and adjust treatment plans as necessary.
\n
• Creating and supporting relapse prevention plans to help clients achieve stable functioning.
\n
• Identifying at-risk patients and collaborating to develop strategies that prevent further deterioration of their mental health.

• Coordinating behavioral health services for patients, ensuring an integrated approach that includes linking with external treatment providers when authorized by the patient.

• Maintaining accurate documentation of patient progress, treatment plans, and communications within clinical records.

• Conducting educational sessions to help patients develop self-management skills for ongoing recovery and resilience.

• Providing valuable consultations to clinic management and team members on behavioral health services, program evaluation, and improvements.

• Adhering to the Code of Ethical Conduct for Clinical Professionals and completing additional tasks as assigned.

Compensation and Benefits\n

This full-time position offers a competitive salary ranging from USD 87,000 to 105,000 annually, dependent on experience and qualifications.
