We are hiring a Nurse Practitioner or Physician Assistant to provide hospital-based palliative care at RWJ Somerset Hospital for patients living with serious illness. What You’ll Do Perform at the top of your license, providing best practice, holistic, patient-centered, evidence-based interdisciplinary palliative medicine. Provide palliative care consultations and follow-up visits across the hospital continuum – from high-acuity ED and ICU encounters to ongoing management on medical, surgical, and specialty units. Manage complex symptoms for patients with advanced illness and chronic disease. Lead goals-of-care and advance care planning conversations with patients and families, coordinating with partner SNFs and/or hospices when appropriate. Partner with the interdisciplinary team and hospital teams, fully integrating into hospital workflows and shaping the palliative medicine practice. Complete timely documentation aligned with clinical, billing, and quality standards. Spend as much time on discharge planning as on intake, managing transitions from inpatient to the patient’s next setting, considering each patient’s longitudinal preference and best interests. What You Bring Active New Jersey Nurse Practitioner or Physician Assistant license and national board certification. DEA + CDS registration, or ability to obtain by start date. 2+ years of palliative care experience required. Palliative care certification (e.g., ACHPN / CHPN) preferred. Strong communication, clinical judgment, compassion, and comfort working on interdisciplinary teams. Compensation & Total Rewards Competitive base salary: $130,000–$160,000/year.
