The role is supported by structured mentorship, physician/APP backup, and a dedicated team. Responsibilities Perform patient‑centered interdisciplinary palliative medicine at the top of your license. Consult and follow‑up across ED, ICU, and medical/surgical units. Manage complex symptoms for patients with advanced illness and chronic disease. Lead goals‑of‑care and advance care planning conversations; coordinate with SNFs and hospices as appropriate. Integrate with the interdisciplinary and hospital teams; shape palliative practice workflows. Document timely and accurately in line with clinical, billing, and quality standards. Balance discharge planning with intake, managing transitions from inpatient to next settings. Qualifications Active New Jersey Nurse Practitioner or Physician Assistant license; national board certification. DEA and CDS registration or ability to obtain by start date. 2+ years of palliative care experience; palliative care certification (e.g., ACHPN/CHPN) preferred. Strong communication, clinical judgment, compassion; comfort in interdisciplinary teamwork. Compensation & Total Rewards Competitive base salary: $130,000–$160,000 per year.
