Key Responsibilities\n

• Conduct comprehensive patient intakes, including diagnostic and functional evaluations, and provide tailored recommendations for treatment plans in collaboration with the primary care team.

• Deliver brief, focused interventions to clients requiring immediate mental health support, while also identifying ongoing needs and connecting them to appropriate services.

• Assist primary care providers in the identification and management of mental health and psychosocial issues, enhancing the overall well-being of the patient population.

• Document patient progress meticulously in treatment charts, ensuring all relevant information is communicated effectively to ensure continuity of care.

• Develop and implement relapse prevention strategies as needed to help patients maintain stable functioning and monitor their progress regularly.

• Engage with community resources and other healthcare providers to coordinate comprehensive care, allowing for thorough follow-up and adjustment of treatment plans.

• Participate in multidisciplinary team meetings to discuss patient outcomes, share insights, and collaborate on innovative treatment approaches that align with clinic goals.

Patient Population / Client Focus\n

Your primary focus will be working with a diverse client base, including those struggling with emotional distress, anxiety, depression, and other mental health disorders. You will support individuals from various backgrounds, ensuring culturally sensitive and personalized care.

Qualifications\n

• A minimum of a Master's Degree in Clinical Counseling or an equivalent field is required.

• A valid and current Licensed Professional Counselor (LPC) license is mandatory.

• Proficiency in mental health assessment techniques, treatment planning, and therapeutic modalities, including DSM-IV diagnoses and behavioral therapy.

• Ability to work collaboratively within a primary care environment while maintaining a patient-centered focus.

• Strong interpersonal skills to establish and maintain effective professional relationships with clients, families, and healthcare providers.

Preferred Experience\n

Compensation and Benefits\n

The position offers a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience and qualifications. In addition to a comprehensive salary package, you will be eligible for a sign-on bonus to support your transition to our organization.
