The ideal candidate will be responsible for evaluating residents, developing individualized treatment plans, and providing high-quality therapy services to help residents achieve their highest level of independence and functional mobility. Responsibilities Evaluate residents and develop individualized physical therapy treatment plans Provide therapeutic exercises, mobility training, balance training, and pain management interventions Monitor resident progress and adjust treatment plans as needed Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure optimal resident outcomes Maintain accurate and timely clinical documentation in accordance with regulatory and facility requirements Educate residents and families regarding therapy programs and treatment goals Ensure compliance with all federal, state, and facility policies and regulations Participate in care planning meetings and discharge planning processes Maintain a safe and supportive therapy environment Qualifications Active Florida Physical Therapist (PT) license in good standing Previous experience in a skilled nursing facility, rehabilitation center, or healthcare setting preferred Strong clinical assessment and treatment planning skills Excellent communication and interpersonal abilities Ability to work collaboratively within an interdisciplinary team Compassionate and resident-focused approach to care Benefits Competitive compensation package Health, dental, and vision insurance Paid time off and holiday pay 401(k) retirement plan Opportunities for professional growth and advancement Supportive and team-oriented work environment Why Join Our Team?
