Work Environment

The work setting for this position is within the home care environment, allowing you to provide personalized and attentive care directly in the homes of patients. You will collaborate closely with a multidisciplinary healthcare team, including occupational therapists, nurses, and other specialists, to ensure a comprehensive approach to each patient's care. This position requires a hands-on approach, fostering meaningful connections with clients as you assist them in regaining mobility and independence.

Key Responsibilities

• Engage in effective communication with patients, families, and all members of the healthcare team to ensure a clear understanding of treatment plans and progress.
• Provide individualized one-on-one patient care, focusing on personalized therapy interventions that address each client’s unique needs.
• Accurately document all aspects of patient care, including clinical progress, billing practices, and correspondence with the healthcare team, ensuring compliance with regulatory standards.
• Utilize advanced clinical knowledge and skills to assess and treat a diverse range of physical therapy cases, demonstrating excellence in managing patient encounters.
• Strictly adhere to all regulatory guidelines, including HIPAA compliance, Medicare rules, and billing and coding guidance, to maintain the integrity of patient information and care.

Patient Population / Client Focus

Your primary focus will include a diverse patient population, including pediatric clients, ensuring that therapy is tailored to meet the developmental and physical needs of younger patients. You will also work with adults recovering from surgery, injury, or managing chronic conditions, emphasizing holistic care and therapeutic interventions that promote recovery and enhance mobility.

Qualifications

• A minimum of a Bachelor's degree in Physical Therapy, with a preference for candidates holding a Master’s or Doctorate degree.
• Current licensure to practice as a Physical Therapist in the state of Florida.
• Proven clinical experience with at least one year of practice in an acute care hospital setting.
• Strong proficiency in trigger point therapy techniques and a comprehensive understanding of human anatomy and physiology.
• Demonstrated ability to work effectively with pediatric patients, adapting therapeutic techniques to their unique requirements.

Preferred Experience

• Experience in a home care setting that highlights your ability to provide therapy in diverse environments.
• Familiarity with electronic health record (EHR) systems for efficient documentation and care management.
• Background in interdisciplinary team collaboration to support comprehensive patient care strategies.
