Endeavor Health is hiring a Mental Health Crisis Case Manager for the Emergency Department in Chicago. You will conduct crisis assessments, determine treatment needs, and coordinate with physicians and care teams to plan disposition. The role involves working with patients, families, and referral sources to assure safe transitions and follow-up.
