Travel RN - Home Health

American Traveler is hiring a Home Health RN Float for a home care agency, requiring 1 year of Certified Home Care and Case Management experience along with an active MA RN license.

Job Details

• Home health setting covering the North Region including Back Bay, Beacon Hill, Boston, Cambridge, Charlestown, Chinatown, Fenway, Financial District, Kenmore, North End, Prudential, South Boston, South End, and West End,
• 13-week assignment with scheduled shifts from 08:00-16:30,
• Weekend rotation required once every 6 weeks,
• Ability to travel to patient homes as assigned,
• Uses Epic for charting,
• Uses UKG/Kronos for timekeeping,

Job Requirements

• Active MA RN license required, no conditional or pending licenses accepted,
• BLS certification required (AHA only),
• Minimum 1 year of experience in a Certified Home Health Agency required,
• Minimum 1 year of Case Management experience required within a certified home health agency setting (hospice, private care, or standalone case management roles do not qualify),
• Flu vaccine required with no religious or medical exemptions accepted,
• 2 professional references required for consideration,

Additional Information

• Responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and families,
• Performs admission processes and case management duties in accordance with home care policies and standards of practice,
• Requires thorough and timely electronic documentation and interdisciplinary communication,
• Start date is contingent on credentialing clearance and may be rescheduled if not cleared by the posted date,
• Local candidates permitted; must reside within 60 miles of Massachusetts General Hospital in Boston for local candidacy,
• No validated parking available at Boston locations; public transportation or paid public parking required,
• RTO must be disclosed at the time of consideration
