Key Responsibilities\n

• Conduct thorough and comprehensive assessments of clients' social, emotional, and psychological needs, using culturally competent methodologies to ensure individual circumstances are appropriately addressed.

• Provide individual and group counseling sessions, employing evidence-based therapeutic techniques tailored to the unique needs of our hospice clients.

• Develop, implement, and regularly evaluate personalized service plans that outline achievable goals for clients, monitoring progress and making adjustments as necessary.

• Advocate for clients’ rights and ensure they have access to vital resources, facilitating connections to community networks and services that promote well-being.

• Contribute actively to program development, implementation, and evaluation, ensuring that all initiatives adhere to best practices and align with our organization's mission.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

Compensation and Benefits\n
