Clinical Social Worker

This position works directly with patients and their families in providing support. Guides families who have financial and housing needs to available resources. Works with state and federal agencies in providing information for placement needs. Identifies custodial issues and refers patients in crisis to behavioral health. Handles referrals and consults from providers in primary care and outlying clinics within scope of responsibilities. Provides encouragement, coaching, and linkage with essential community services that support each patient's unique healthcare needs.

Key Essential Functions and Accountabilities:

• Interviews patients or their designees to obtain relevant psycho-social information.
• Determines the effect of illness upon the patient and the patient's family.
• Participates in the development of an interdisciplinary plan of care.
• Coordinates the discharge planning process and makes referrals for appropriate home or social services care and equipment or supplies.
• Provides therapeutic support and education to patients and families.
• Assists patients and families in understanding medical recommendations.
• Makes interventions and professional support referrals as needed.
• Participates in interdisciplinary team meetings and patient care conferences as necessary.
• Manages an assigned caseload.
• Documents results of psycho-social assessments and plans in the patient's medical record.
• Ensures documentation is timely and meets regulatory compliance and departmental standards.
• Performs other duties as needed to help drive the organization's vision, fulfill the mission, and abide by values.

Knowledge and Skills:

• Demonstrates an understanding of sound ethical practice in conjunction with established ethics of training, discipline, and education/license level.
• Has knowledge of basic nutrition, physical activity, and tobacco cessation recommendations for health and disease prevention.
• Applies case management principles in integrated settings using tact, discretion, initiative, and independent judgment.
• Demonstrates customer service and recovery skills.
• Possesses knowledge and awareness of community support resources.
• Skills in communication, reporting patient activities, and deviations from normal.
• Ability to interact positively and effectively with patients and families.
• Proficient in using equipment and software such as computers and electronic health records.
• Applies effective oral and written communication skills.
• Exhibits empathic and supportive listening.
• Develops interpersonal relationships to work as an effective team member.
• Ability to maintain strict confidentiality of patient care information.
• Maintains documentation in compliance with regulations.
• Utilizes professional communication and conflict resolution skills.
• Can multi-task and work independently in a fast-paced, dynamic environment.
• Maintains compliance with annual competencies.
• Provides advocacy and outreach.
• Problem-solves and uses conflict resolution skills to develop and maintain good working relationships with staff and departments.
• Meets patients' needs in a prompt and appropriate manner.
• Works with individuals from diverse backgrounds, including those dealing with addictions and severe mental illnesses.
• Uses the computer system to obtain and document patient information.
• Effectively coordinates with a multidisciplinary team in accomplishing treatment plan goals.
• Works collaboratively with state and community agencies on behalf of patients.
• Organizes time, responsibilities, and accomplishes tasks by established deadlines.
